The Plain Doll
The Plain Doll usually waits on the steps in the Hunter's Dream. Gaining your first Insight unlocks leveling; if Insight later returns to zero, she becomes inanimate but you can still spend echoes through her. When she is absent from the steps, check the grave behind the workshop.
I separated the mechanic from the metaphor. The mechanic is: gaining your first Insight unlocks leveling permanently, even if your Insight later returns to zero. The ending image, in Childhood’s Beginning, is the Doll holding the newborn. I will not write that she “is” a Great One. The sources do not say that. They say she is there, and that she is the one who levels you.
Unlocking and using levels
Encounter a boss or use Madman's Knowledge to gain the first Insight, then interact with the Doll. You do not need to keep an Insight point in reserve after that initial unlock. Spend Blood Echoes before a risky trip if you do not want to risk losing the carried amount.
The grave and the ornament
Her position and idle behaviour depend on game progress and random states; carrying a large pile of echoes is not an established trigger for the grave visit.
Bring the Small Hair Ornament from the Abandoned Old Workshop and give it to her for a Tear Stone. Her reaction and the Old Workshop are direct clues to her past. Her visual and vocal connection with Lady Maria invites interpretation, but it does not establish that the Doll is a Great One.
Questions
Why will the Doll not speak?
At zero Insight she is inanimate. Gain your first Insight to unlock leveling; after that first unlock, you can still level through the inanimate Doll at zero Insight.
Where did she go?
Check the grave behind the workshop. Her visits are linked to progression and random behaviour, not a verified threshold of carried Blood Echoes.
