Bloodborne lore, carefully
Yharnam is famous for blood ministration. Scholars at Byrgenwerth found old blood tied to the Great Ones. Laurence left and founded the Healing Church. The church’s blood healed, then turned people into beasts. Hunters were the answer, and then part of the problem.

I will not write this as a solved timeline. Wikipedia’s plot and the wiki’s chronology page disagree in the details, and the wiki labels its order as theory. What I will stand on is the public plot: you are an outsider who signs a contract, wakes in the clinic, and is told to seek Paleblood. The rest of this section links the arguments instead of pretending they are patch notes.
The parts that are plot
The hunt takes you through the city to the Grand Cathedral, then to Byrgenwerth and Rom. Rom's death reveals the red moon and changes Yahar'gul. The One Reborn, the Nightmare of Mensis, Micolash and Mergo's Wet Nurse follow. At the Dream, accepting Gehrman, defeating him without the three-cord condition, or defeating the Moon Presence after that condition lead to three distinct endings.
Factions you will hear named: the Healing Church, the Choir, the School of Mensis, and the hunters’ workshop. Item descriptions do the real storytelling. A wiki summary is one reading of those descriptions.
Questions
Is there an official timeline?
No published year-by-year timeline from FromSoftware. See the timeline page for what is plot and what is theory.
What is Paleblood?
Miyazaki's Future Press interview offers two supported readings: the sky after Rom and another name for the Moon Presence. The Paleblood page separates those clues from broader fan interpretation.
