What Paleblood is not settled
Paleblood has two developer-supported readings: the pale sky revealed after Rom and another name for the Moon Presence. The clinic note sets the goal, while later notes and Miyazaki's guide interview add context. That does not make every detail of the hunt a settled explanation.
The distinction here is between game evidence, the creator's explanation and further interpretation. The Future Press interview is available as a community transcription; its two readings deserve more weight than an unsupported theory that Paleblood is simply a special medical blood type.
The sky and the hidden ritual
In the Future Press interview, Miyazaki connects Paleblood to the drained-looking sky after Rom and the revelation of the Mensis ritual. He also points to the earlier sky message in Yahar'gul. In that reading, seeking Paleblood involves uncovering and confronting the hidden ritual.
The Moon Presence
The same interview gives a second reading: Paleblood is another name for the creature that descends from the moon under the ending conditions. Miyazaki points to a Lecture Building message as a hint, while deliberately leaving room for interpretation.
What remains interpretation
The opening clinic note, the later messages and the interview are evidence; claims about every motivation, blood lineage or symbol still require their own support. For the gameplay route, consume three cord thirds before defeating Gehrman, refuse his offer, and defeat the Moon Presence. A lore interpretation is not a substitute for those ending flags.
Questions
Is Paleblood only an unexplained word?
No. The game offers later hints, and Miyazaki's Future Press interview discusses both the sky after Rom and the Moon Presence. The wider meaning still leaves room for interpretation.
Is Paleblood the Moon Presence?
That is one interpretation explicitly supported by Miyazaki in the guide interview, not merely an anonymous fan guess. He also discusses the sky and the revealed ritual.
