All three endings
Yharnam Sunrise is submission. Honoring Wishes is refusing Gehrman and winning. Childhood’s Beginning adds three consumed One Third of Umbilical Cord items and the Moon Presence. Each ending has a trophy.

Choose your ending route
Spoilers for all three endings. Answer for your current playthrough. Consuming at least 3 Third Umbilical Cords before defeating Gehrman is the condition; holding 3 in your inventory is insufficient.

- Yharnam Sunrise: submit your life. Cords do not change this choice.
- Honoring Wishes: refuse, then defeat Gehrman with fewer than 3 cords consumed.
- Childhood's Beginning: refuse, consume at least 3 cords before defeating Gehrman, defeat Gehrman, then defeat Moon Presence.
Ending the game starts NG+. Complete other exploration and the DLC before making your final choice. Cords do not carry into the next cycle.
I wrote the cord rule in one sentence and then checked it against the boss page. Four cords exist. Three must be consumed. The consumption counts if it happens before Gehrman dies. You do not need a perfect no-death run, and you do not need every ending on the first file. NG+ exists.
Decision
After Mergo’s Wet Nurse, return to the Dream. Gehrman offers to end the hunt. Accept, and the sky becomes sunrise. Refuse, and you fight him. If three cords are consumed, the Moon Presence replaces the end of that fight.
All four cords are in the base game, not the DLC: Mergo’s Wet Nurse, the altar in the Abandoned Old Workshop, the impostor in Iosefka’s Clinic after Rom, and Arianna’s newborn after her quest progresses. The last two depend on NPC survival and timing. You need to consume three, not merely carry them; collect them before finishing the Dream.

How do I get the true ending?
Consume three One Third of Umbilical Cord items before Gehrman dies, refuse him, beat him, then beat the Moon Presence.
Can I get all trophies on one playthrough?
One uninterrupted route without restoring a save gives one ending. To earn all three ending trophies in the same NG cycle, back up a save before the final choices and restore it for separate branches; otherwise use later NG+ cycles. Keep the pre-consumption save for the Honoring Wishes branch.
